Health Assessments for Aboriginal and Torres Strait Islander Patients
Our team conducts comprehensive health assessments every nine months for Aboriginal and Torres Strait Islander patients, known as the 715 health check. Health Assessments for Patients Over 75
We offer detailed health assessments for our patients over 75 to help manage multiple co-morbidities. Yearly CHAP Health Assessment for Patients with Intellectual Disability
We provide annual Comprehensive Health Assessment Program evaluations for patients with intellectual disabilities.
